This was a very straight forward employment application. The only problem I had with it was there was no space for a birthdate.

We just started using these applications for a new coffee house. They seem easy for applicants to fill out without questions. The one section I found was missing was availability. I just had them put that info in the remarks section on the back. We plan on continuing to use these applications. The one big thing I did like was they are on a single sheet of paper, I am not a fan of the 4 page applications.

The form is very general in nature and if that is what you're looking for then this is perfect for you. I didn't need an application that went very deep and this worked for us.
